Having a chronic disease can be debilitating and we want to help you get back to optimum health as quickly as possible. We will help you manage your medical condition and get it under control through health assessments, home medicine reviews and GP management plans.
We may be able to facilitate referral and possible Medicare rebatable services to allied health services such as dieticians or physiotherapists to help manage your condition better.
This is an important update about MyMedicare and Care Plans.
Some Care Plans (including Chronic Disease Mangement Plans and Mental
Health Care Plans) can only be completed at the practice a patient is
registered with under MyMedicare.
At this time, Market Street Medical is not registered with MyMedicare.
If you have previously signed up to MyMedicare at another clinic - even if you
may not remember doing so - your Care Plan and allied health referrals will
need to continue through that clinic.
If you are not registered anywhere under MyMedicare, your Care Plan can continue to be completed here as normal.
